Learn about the development and implementation of Advance Care Planning in Canada – A Capacity Building Initiative. Discover new tools and resources designed to support health care professionals and others to encourage and facilitate ACP among all Canadians, including:

  • Current attitudes and awareness about ACP in Canada
  • Updates to the Pan-Canadian Framework and current priorities driving change
  • Toolkits and guides to assist with ACP in LTC, home-based and community care, and more
  • Campaigns and outreach efforts to increase awareness and support for ACP

Featuring: 

Karine Diedrich is the Director of the Advance Care Planning in Canada project with the Canadian Hospice Palliative Care Association (CHPCA). She has worked in the non-profit sector for 15 years, leveraging skills in partnership development, strategic planning and communications to engage diverse stakeholders across sectors to develop innovation solutions to new and ongoing communications and engagement challenges. She has a BA in Criminology from Carleton University and a Professional Certificate in Partnership Management from the Norman Patterson School of International Affairs (NPSIA).
